Welcome to CMC’s Advanced Care Transportation Service
Our Advanced Care Transfer program allows, Emergency Department to Emergency Department Transfers, Direct Admits from Home or Physicians Office and Inpatient Transfers of a patient for specialized emergency and non-emergency care providing immediate access to the community 24 hours 7, days a week throughout the Northeastern Pennsylvania Region.
Program Features:
- Easy Access to CMC Services and Resources
- Rapid Response, around-the-clock coordination to expedite patient referrals from outlying facilities
- Nurse Coordinator for referral & follow-up communication
- Referral is complete on day of call
- Physician Specialist available 24 hours a day, 7 days a week for quick authorization of hospital-to-hospital referral
- CMC will arrange expert ambulance service in timely manner with local ambulances and air transport system
- Emergency Department to Emergency Department Transfer
- Direct Admits from Home or Physician’s Office
- Inpatient Transfers (Emergent and Non-Emergent)
